Alberta Health Services is preparing to launch Connect Care Wave 3 in 31 facilities in the west side of the North Zone during the weekend of April 10-11, 2021.
The 31 sites will be fully or partially launching in Wave 3, the sites include the following;

- Fox Creek Healthcare Centre
- Grande Cache Community Health Complex

- Grande Prairie College and Community Health Centre
- Grande Prairie Lab Services
- Grimshaw Berwyn and District Community Health Centre
- High Prairie Health Complex
- Hythe Continuing Care Centre
- La Crete Community Health Centre
- La Crete Continuing Care Centre
- Manning Community Health Centre
- Manning Medical Clinic
- Northwest Health Centre, High Level
- Paddle Prairie Health Centre
- Peace Diagnostic Imaging, (Grande Prairie, La Crete, Peace River and High Prairie)
- Peace River Community Health Centre
- Queen Elizabeth II Hospital, Grande Prairie
- Rainbow Lake Community Health Services

Connect Care is a information system that collects health information around the province to improve and provide consistent healthcare practices. Albertan’s health records from AHS, its subsidiaries and partners will be accessed through one common system.
The province currently has around 1,300 independent systems, and with Connect Care the entire province will be connected together to bridge gaps between healthcare providers in different areas, patients and their healthcare information as well as healthcare now and in the future. Patients will be able to seamlessly move between care providers, services and facilities with the new system.
When fully in place, Connect Care will be wherever AHS provides care or is responsible for a person’s record of care, including hospitals, AHS clinics, continuing care facilities, cancer centres, addiction and mental health facilities, AHS-run community health sites, Carewest, CapitalCare and Covenant Health sites, Alberta Precision Laboratories locations, and DynaLIFE locations.
Connect Care has been launching in phases around the province, with nine implementation phases which starter in 2019 and is set to be complete in 2023.
